My guess is you took a picture of Penny at the stairs, then took a seperate picture of Sheldon, and put him in here, and changed the look of the photo?
I can tell it's not a real drawing because Sheldon and Penny have different reflections from the light and Sheldon isn't putting any shadow on Penny, while she's giving a shadow on the stair's banister.
