You put a positional light into the bulb and set the vol. light to the number you used for the pos. light. It can be a trial and error to get them right.

Took some movements like you said, just needed to MC to cooperate. Learned from previous and was happy to get rid off the light spots, altough it was a very fast 4k render.
It is neccessary to use soft clipping and exposure in order to get lights right
