If you're wondering, this picture takes place when Blackstar (Who is the fnaf 3 night guard.) Comes back for a 7th night. He dies shortly after inside the 'Gato' suit.
This isn't a spoiler sense...well...It was in one of my first videos. (Don't watch it it's crap, it also doesn't link well with the later changes. So it's no longer cannon. I'll make a new version sometime soon that not only is cannon but will fit better.)

I blame it on Lord death reaper chopping him so often. (Blackstar gets so much trauma to the head I'm surprised there's nothing wrong with him.)
But if we're serious, he went back because
1. PB has star eyes, and stars are obviously linked to Blackstar's family.
2. Curiosity about the murder at Freddy's.
But in this situation he should have taken someone with him. So yeah he is at fault, knowing Spring was there and that he could be killed. He took a risk and it didn't pay off.
