Amazing fanart! You must love to play the game, because i see a damn big lot of collected items on that picture (i'm not even sure all of them would visually combine that way though, but in this case it doesn't really matter - and aside from that they are mostly very true to the ingame-appearance).
Holy crap, it looks like you included pretty much all appearance-changing items!
Even those which give you a happy face after taking a closer look. You probably didn't change their mouths to create a more unsettling effect. Which is probably also why The Sad Onion is the only item seen four times
Let's see if i can find them all:
Isaac: Mom's Heels and Underwear, The Belt, Rosary, The Blood Bag, The Sad Onion (this one might be Max's Head though), The Battery, Wooden Spoon, Wired Coat Hanger, The Mark, The Pentagram, Magneto, Halo, Super Bandage (without Bandage Girl's flower)
Judas: Growth Hormones, Technology, Skeleton Key, Blood Of The Martyr
Cain: <3, Boom!, The Inner Eye, Fetus in A Jar (i was about to say the top hat comes from The Dollar, but that's not the case 
???/Blue Baby: Steven, Fly Love, A Rock, Cupid's Arrow
Maggie: Mom's Lipstick, Money Is Power, Spoon Bender, Mom's Contacts, Loki's Horns, The Parasite
Eve: Steam Sale, Charm of The Vampire (like Roid Rage shown very subtle), Ouija Board, X-Ray Vision, 9-Volt
And then there's also that Familiar-sized Whore of Babylon character that might be a Demon Baby.
It seems you also put various items with purpose on a certain character (X-Ray Goggles with Ouija-Board-face on Eve, lipstick on Maggie, poop on ???, and the monocle over Cain's eye-patch.)

Wow, this really is one of the best fan arts I have seen for the game. Like Pikachu29 said, it really captures how distorted and mutilated the characters become as the game progresses. Amazing work!
