Story behind it-

Ulquiorra and Orihime are married for a few years, until Orihime gets sick. Her sickness gets worse to the point where she passes away. They do promise to be with each other, even in the Soul Society.
Other things happen to where he's able to walk the SS as a human spirit.
Ulquiorra was able to do the impossible and actually find her, even though they're considered family, and family members hardly find each other in the SS.
He did have some help. coughcoughAcertainshortshinigamiwealllovecoughcough.
Anddd Ulquiorra pretty much tackles Orihime, when he finds her. Some amazing feat, she remembers him, leading to this.
