Its an artificial jaw, Ala Raiden from MGR. Thought it would serve as a good attachment point for his mask as well.

I've always wondered how people like Darth Malak and Raiden manage to speak with those prosthetic jaws...

I can't speak to Raiden's vocal abilities, but I believe Darth Malak does reveal that he speaks with the aid of a vocal synthesizer prosthetic in a cutscene.

With Raiden, it's clear that his lips are moving. His jaw isn't completely destroyed. It's more likely bolted-on jaw protection.
As for Genji... It think it's a bit of both. His vocal chords and Jaw were damaged in the showdown with Hanzo. The Metal might simply be reinforcement to a now fragile jaw, and instead of a whole synthesizer, it's an amplifier to a damaged voice.
That's why he can't eat properly. The Amplifier around his throat is too bulky to allow for large swallowing. Purely liquid diet.
