There are multiple different worlds as well as alternate planes of reality. These planes of reality aren't separate and parallel to each other; they intersect each other in multiple place. This in turn creates places that are less stable and susceptible to random portals that can pull people into other realms. The more planes intersecting in an area, the more unstable it is (such places like the Bermuda Triangle). The three planes that intersect with the most planes are Felarya, Heaven, and Hell. Because of this, those planes not only get the most portal travelers, but also the most dead. This might need more clarification. When a person dies, they are randomly sent to another plane of reality that connects with their plane. This could happen a few different ways. If a person dies young or suddenly, that person could be sent to another plane with the same body he had and with his memories still in tact (but this doesn't always happen). Sometimes they wake up in another plane with their memories intact, only they are a different species, maybe even a different gender. But often times, they could just be reborn completely in another plane of reality. But since Heaven, Hell, and Felarya have the most intersections with other planes, most of the time a being who dies ends up in one of those three places. Where they end up is random. Contrary to the belief that good people go to Heaven and evil people go to Hell, it could end up the exact opposite. There have been unfortunate nice people finding themselves in Hell after death. Though there still is a good reason not to be evil because all the evil souls that randomly end up at Heaven will be discovered quickly and thrown down to Hell. Heaven has it's own personal portal to Hell that only goes one way. Think of Hell as Heaven's trash can. Unfortunately for the good people that randomly end up in Hell, well, Hell doesn't like giving people up.
Also contrary to belief, demons aren't all evil. They are like humans: some are good and some are bad. They had lived in their realm just fine until heaven threw down a bunch of rebel angels down to Hell as punishment. The leader of the Fallen soon took this as an advantage and took control of the demons that originally lived in Hell. He rose to power and gave demons the gift of immortality like the angels. There was only one difference, demon immortality isn't true immortality. They all have to preform certain tasks in order to stop aging. If they do that task, they keep their immortality, but if they don't do said task, they age a day for every day they don't do it. The different races of demons were created out of what tasks they have to preform. One example is incubi and succubi which have to cause one person to climax every day to keep their immortality. Other races of demon have to do crueler things though. What task you get depends on what tasks your demon parents have.

Suffering didn't like her role in Hell. Like Pain, she had to torture the souls in Hell that weren't allowed to die so they can suffer eternally. Because people that die end up her randomly and unlike Heaven which tosses away it's sinners, Hell keeps the innocents that unfortunately end up there. She did her best to only torture one person a day to keep her immortality, but would never harm an innocent or the sinners that have truly repented after centuries of torture. When she ended up with one of these people, she did not torture a person that day, because it was better to age a day instead of hurting someone who doesn't deserve it. She even hated torturing the sinners. But demons are born with their roles and can not change them.

Then she heard about Pain and how he escaped into a world where demons didn't have to fulfill their roles to keep their immortality, a whole world that is filled with so much magic, people age so slowly that they seem immortal. Suffering left Hell for Felarya and changed her name to Mary. She vowed from then on that she would not be the cause of suffering anymore. She will help people from now until the day she dies.
