A few years after Alex was born, Rouge and Shadow were on a mission when Shadow suddenly collapsed in pain. They found that at some time in the past, Eggman had implanted a device inside of Shadow to prevent him from using his Chaos Abilities should he decide to join Sonic's side, but must've forgotten about and never used. However, time had worn down the device and self-activated it.Shadow's options were never using his powers again, or surgery to have the device removed. He chose the latter, and Rouge promised him she'd be there when he woke up...
A little thing I thought up the other day.
The flowers and card are from Sonic. Since Shadow took care of him when he was in the hospital back in Shattered Glass, it was Sonic's turn to do something nice for Shadow.

I'd think with all of the experimentation done on him in the past, Shadow would have a problem with being around doctors and people in lab coats. Rouge knows this, and stays with him to help him feel safer and reduce stress about the situation.
