Honestly I wonder about that a lot too. I haven't managed to get inside Sans' room in the game but when I do (because I fully intend to) I want to inspect it and see wtf is up with Sans having something like that. Especially since he doesn't seem to have a lot of expensive things in his room.

Actually no. I had to look up if you ever see inside Sans' room and although I'm trying to avoid spoilers as much as possible it turns out getting into Sans' room is, not insanely difficult, just really really cryptic. (what a shock for Sans, right?)
You can only do it in a pacifist run though. And I don't think it works if you've ever done a genocide or no-mercy run before doing a pacifist run.
