Hi!. I think you should try to use more soft brushes in wide areas, and more hard ones on the smallest areas. Don't be afraid of using color, and blending in.
For example, in the shadow, use a soft brush, with low opacity.
Keep practice, you're doing good.

much depends on the subject, and knowledge you have of it; also another point to consider is the complexity of the image. For me personally if an object and not a portrait, for a semi-realistic version I can take from 45 min to 2 h. But it is not an exact time, also the inspiration of the moment plays an important role.

How do you create the colour palette? Do you use the colour-picker tool to pick the colours with photoshop or do you just do it by eye?

In this particular case I took the main colors from the reference image with the "color picker tool" but some extras like shades of shadow and highlight those elected by eye.
