I's a very good question. It all bottles down to the ooze/mutagen used on them, since they are rule 63 tmnt characters, their original counterparts origin story can be implemented in here. The old cartoons version of the mutagen, mutates any living being into a cross hybrid between the last different species they had contact with. In the comics and the new animated show and live-action movies, the ooze mutates creatures into a humanoid state.

In this fan creation version the ooze could go even further giving animals human features, and not only humanoid features. Thus giving them hair and mammary glands or simulate mammary glands. Since Turtles can't take off their shell, the breasts are for estethics only and serve no purpose as the original intent are among mammals.
