In survival mode, the sleeping bag and the mattress can't be slept on more than 3 hours and 5 hours at a time respectively, and does not allow you to rest completely. You wake up tired with a reduced refresh rate for your action points. So you need to find and consume a lot of caffeinated drinks like Nuka-Cola

Seemed to me more realistic to use mattresses and sleeping bags at the beginning of the game unless you can use an existing bed. Building beds with those steel frames at the start didn't feel right.

Bad sleep quality is one of the causes for contracting diseases. Better to look for and use those herbal remedies.
