I got mine here: [link] It looks like most of the suppliers are in the UK, but there's probably some equivilent in the USA (also, I'm not sure if there's something different over there that's also called plastazote, I found several references to medical equipment looking for it).

I've found some types of foam camping mat can be used as a substitute, including some I got at £1 shops, but you're probably best checking craft stores. You're basically looking for a quite thick (about 6mm) foam that can be easily cut with a craft knife. The advantage of plastazote is that it can hold a shape well on it's own once it's glued together, but if you have a less rigid foam the cardboard should be able to support it.
