Bulbasaur are native to Kanto, with occasional sighting in the wild in Johto and Kalos region. Although not properly documented, some Bulbasaur has shown different spot patterns on its forehead. The exact factors for the occasional differing pattern has not been identified yet, although random mutation can play a part, much like in the case of Spinda.

Inspiration from the anime, specifically the Bulbasaur owned by May that debuted in the episode Grass Hysteria! (AG073).

I can imagine this pattern to be exclusively female only with Gentle Nature, to reflect on the personality and gender of May's Bulbasaur.
