Well, I guess it's mainly about having patience, and going at it through trial and error - oh, and a good idea of what you want to achieve before you start.

The key is a good base UV map of your model (as 'flat' and as 'logically' laid out as possible - try matching the UV map seams with how they would appear in real life - e.g. down the sides of trouser legs).

Then using your drawing software (I use Photoshop), create a new layer. Flood fill this with a basic 50% grey colour.

Then create a further new layer and with a soft brush (with shape dynamics - I use a graphic tablet and pen) draw some creases with a 100% black colour - brush size and settings will depend on your base map size and the item you're creating - I try to make my base map size 2048x1536 - which gives plenty of scope for details.

The creases should try to mirror how they would appear in real life, i.e. they appear around the 'stress' point (e.g. seam joints, where the material is stretched etc) of the fabric.

Then on another new layer do the same thing but with a 100% white brush and run lines in between the black creases you've just drawn. This will give you the 'ups and downs' of the folds etc.

When you're 'happy' with the creases, you can vary the layer opacity until a fairly soft contrast is acheived, then flatten the layers.

I then use a small amount of gaussian blur just to soften the edges, and then change the image to a greyscale one and play around with the contrast/brightness until it looks reasonable (try squinting at it and if it looks like cloth then okay!)

Then its around playing with the bump and material setting on your final renders, to make sure that the lighting picks out the details you've created.

Don't worry, if you become obsessed with this! - I've followed many people in the street just looking at their clothes to see how the folds are produced as they move etc. and got some very funny looks of people!!
