This is from the second Star Trek pilot, "Where No Man Has Gone Before". Beside the different uniform shirts Spock's appearance is more severe than it was in the first pilot "The Cage" and the subsequent series. Spock's shirt also has a unique collar that opened on the sides to save on the wear and tear of Leonard Nimoy's makeup.
