Take a look at the offcial art of Sheik. Just look at that package! (Here, for example: [link] ) It's not necessarily a very impressive package, but a package nonetheless. And I doubt Zelda would want to make herself very well endowed. In those tights, it would attract far too much attention. ^_^

I'm pretty sure Red Ninja was all about the sex appeal. That's just an example of realism and practiality being completely absent.
A noticeable bust on Sheik wouldn't be an improvement on his character or need to remove unwanted attention. It would only serve as fanservice. Much like Tifa, the top heavy monk in FFVII.
Of course, it's only fanservice for the fans that actually see Sheik as a woman >.>

Anywho the "2+2" guideline is just my explanation why there's so much fan-created doujins, fanarts, fics, yadda yadda yadda; all the way till that line between canon and self-satisfaction is already whacked and confused since there are no official explanations.
That "official" Zelda manga didn't help the case either. (albeit, it's full of plot-holes)
As to your last point, your not one of those people who's going to yell at me for making a stupid joke are you ._.

I dunno.... Zelda, even when she was grown up, didn't seem all that busty... and since Sheik had to be doing all those fancy ninja scurry-away-in-the-nanosecond-that-Link-spends-blinking tricks, I doubt she would have wanted what little boob she had flapping around in her face, so she'd have to do SOMETHING to restrain them.
So, with that in mind, Sheik's chest does look exactly like Zelda's would if she had to disguise herself as a Sheika for seven years or whatever (I don't know about you, but when I see Sheik, I see a little lump across the chest that could be one of a) chickboobs and an ace bandage or b) the manboobs of a really-toned-yet-skinny guy (iow; Sheik) .... and since she WAS passing herself off as a male, it's only natural that we'd assume the last one.
Though I hear that wearing boob wraps too often and too tight can repress their growth or make them stay compressed for weeks after you take the wraps off D:

Well, considering that this red ninja of yours was barely clothed, and what little she had was bright red (all great ninjas know that red makes you hard to see) , I highly doubt that the creators gave a rat's ass about what was convenient or helpful, as long as it excited their weasel.
Perhaps she seeks to distract her opponents by showing them her breasts every time she spreads her arms, or she hopes that they'll bleed to death from the bloody nose. Plus, she'll have no problem finding the rapists and sex offenders so that she can beat them up.

Realistically, however you could never hope to fight successfully with your boobs flying all over the place. I mean, come on, it hurts like a bitch when they bounce like that, and, assuming you have a size B or above bust, they could very well throw you off balance.
Besides, Sheik was supposed to be a man. Having breasts bulging out of his chest would have made it rather difficult to convince people of that.
