Can't quite see the top of her hair, nice pose, might make her shoulders a tad wider.

The missing top of the hair is more a style choice, indicating that she's got some heavy light coming down on the top of her head. I see what you're saying about the shoulders. Thank you for the input, by the way.

I usually make them up. However, in some cases, the commissioner lets me know a specific type of clothes he's looking for. They may give me specific reference pics to draw from or I'll take their clothes description and look around on the internet for the clothes that best fits their description.
