During the What If episode, Peggy found herself subjected to Erskine's treatments, a specialized serum and exposure to a unique form of radiation, and realized the professors dream...but it didn't turn out the way as suspected.
Not only did Peggy becoming the world's first, fully developed super soldier, but Steve's head (skinny version of himself) grew onto Peggy's shoulder making the two conjoined together, now controlling Peggy's right side of their new developed body.
Not only did Peggy became captain Britain and infused with the super soldier serum, but herself and Steve becoming the first ever two headed super soldier.
