With the pen?
I had that problem too, so I messed around with the settings. If it's on SAI, put the settings to something like this:
Min Size 0%
Density 100
(Simple Circle)
(No Texture) <-- Ignore those two
Qualty 4 (Smoothest)
Edge Hardness 100
Min Density 100
Max Dens Prs. 100%
Hard <-> Soft 0
