Yes. He always looks like this. XDLiterally posted these in reverse order. Alive was drawn last, Little Off the Top was drawn second, and this headshot was drawn first.
While Deadchan is capable of returning from the dead, it can take time for certain injuries to heal entirely, sometimes even leaving scars.
Because of this, he rarely stays alive long enough to heal entirely, and usually ends up starting the whole process over again.
A friend of his is constantly tending to his wounds, no matter that he'd been fine without treatment; they insist. Helps that said friend is in medical school, and is probably just using him for practice.
