The Doctor said he never referred to himself as The Doctor during the Time War, therefore it doesn't count as an incarnation of the character titled "The Doctor", we simply call him the "War Doctor" because he never gives himself a new name. The High Council of Time Lords called him "The Renegade". The 10th Doctor regenerated twice, first time to create the Meta-Crisis doctor during Series 4 and then into the 11th Doctor in The End of Time.
