MTG = Magic: The Gathering, card game taking place in a multiverse of pocket universes.
Innistrad is one such plane, full of werewolves, vampires, typical gothic horror stuff, etc. etc.
Recent Events on Innistrad: an Eldritch Horror by the name of Emrakul has been "summoned" to Innistrad. Proceeds to Eldritch Horror the shit out of everything.
And here we see what happens when a rider fuses with his horse, and is completely reconstructed with tentacles.
That's the rough idea anyways.

And I freaking love it, as well as looking to be a busted card, the flavor and the name - THAT name! - just ooze sublime Eldrazi
