How do you put the pictures in the blanks?

Easy. Open up the desired locomotive in a separate MS Paint window, click the 'Resize' button, click the 'Pixels' button and type in the desired measurements, then click OK, click Select and highlight the whole picture, click 'Copy' and paste picture on the window with the meme.
