GIMP! 
I do a lot of changing up the brush size and how fuzzy it is. And the opacity. That makes it a lot easier to do fine detailed work with a clone tool. It takes me about an hour for each one of these... but I don't count the time spent finding the pictures that are easy to work with. That takes forever.
Mostly, it's practice. I'd be happy to do an artstream sometime to show off how my little tricks work in practice. Drop me a note to tell me when, or just wait 'till I post a journal announcing it.
