Well....

Mining in the caves, she finds an opening. Using up the last of her torches, she uses the potion, not knowing it was a breast expansion one, instead of night vision one. She walks over a tripwire and activates a trap. The trap goes off, flattening her into the stone wall. Once the trap is finished, the potion takes effect, making her front grow out.

Lets see... Cave... Flattened... Potion... Big boobs (Yay!)... Wall behind her looks slightly out of place... Perhaps she drank a potion she found in a cave but triggered some sort of trap, crushing her before the potion took effect?
