The chest scar is supposed to be on The Boss, not Big Boss. The Boss gave birth on the battlefield, via a messy caesarian section that left her with a long, snaking scar across her abdomen. (Spoiler: Ocelot).
Aside from that, and the sword being upside down, it's good.

He has a scar like this in Peace Walker. There's more to it but I don't wanna spoil the game for you; but it's definitely there, so this is not a mistake.

I have no idea when they talked about it, but when he's running around half naked the scar is pretty obvious.
