As I mentioned, strangely enough, the picture isn't upside down - it's on its side (the cigarette's pointing downward).

The picture's intended to make the viewer lose their sense of direction, hence attempting to help convey one of the picture's subjects - a lack of emotional direction (aka, a feeling of "floating in space").

Dunno if any of that's of interest, but there it is.
