Being turned into a girl in the middle of class due to a prank could do that.
Edit: Now in color...although in reality the tights are sheer-burgundy but didn't come out as well I might post that variation later.

If they started out as a guy at, say, 150 lbs, SHE'D have to end UP at 150 lbs...

There is also the possibility that excess mass is burned up to provide the needed energy for such a transformation. Thus the resulting second state would always be lighter than the first state.
This of course would mean that to return to the first state one would have to become overweight, else the mass loss would be critical most likely leading to a long hospital stay or even death. That is, unless the resulting return would result in an even smaller first state.
