I always figured it went back to their childhood...

The turtle tots would emulate what they saw in comics and on tv and want to play pretend.  There's usually a hero, a sidekick, a villain of course, but most importantly, a damsel in distress! I figure Donnie and Mikey would usually get stuck playing that part and then eventually would demand to know why Leo and Raph never had to play "the girl", Leo would easily say that it's because they were older, and Mikey would say that's not a good reason and Donnie would agree. Smug Raph would then add that it's because they are taller and everyone knows that Boys are taller than girls- thinking back at all the pictures in comics and movies on tv they have seen, Donnie and Mikey would sadly concede to that fact and Raph would be all high and mighty about it. Leo would feel bad at seeing Mikey's disappointed face, so in order to make his baby bro feel better he would promise that if Mikey ever got taller than him, then HE would play the girl and Mikey could be the hero- to which Mikey would then instantly cheer up.  Donnie wouldn't say anything, he would just look at Raph shyly, hopefully, and Raph would be like "what?" so Leo, sensing Donnie's unasked question, would ask it for him; "What about YOU Raph?"  "What ABOUT me?"  "Well, what if Donnie gets taller than you?"  "HA! Yeah right! The day 'scrawny Donnie' is taller than me is the day I prance around like a fairy princess! " Mikey adds in "...really?"   "Sure, and pigs will fly and you'll grow a brain!"

....Years later, Splinter finally agrees to let them enjoy their first Halloween, so April gets their sizes and raids a thrift store, unfortunately there are only a few costumes that will fit over their shells, and one of them happens to be a pink dress... Mikey must have grown a brain because he remembered something "Hey Raph, remember what you said when we were kids about Donnie ever getting taller than you?" *wide grin*
"what're you talking about shell for brai- OH. Ohhhhhh NO!" *eyes widen in horror*
Leo get's his smug smile that says he knew this would happen and karma is a bitch. "A ninja always keeps his word."
"sonofa-!"
Donnie, seemingly ignoring the onslaught of upcoming curses, then innocently turns to April and cheerily asks if she would happen to know where to find a crown and a pair of fairy wings.
April just stares at them cluelessly for a minute before replying "sure."

As much as he would loathe it, Raph is a turtle of his word, so he would suck it up (and add the promise of beating the green off of anyone who laughed) Splinter would find it amusing and chalk it up to a lesson about choosing one's words carefully, making sure you find them palatable,  for one day you may have to "eat them" so to speak.

So Raph decides if he has to be a f**ing fairy princess, he's going to own it and be the most bada$$ fairy muther f***ing princess you've ever seen.

Raph in a dress = owns it

Mikey in a dress = acts flirty and keeps posing

Donnie in a dress = a confused and blushing turtle

Leo in a dress = wonders why girls wear it
