It's a chibi, chibi style is cute.
It's a self portrait, they have scars and they often have nosebleeds.
This is what they look like, just because they have scars doesn't mean people should never ever call them cute.

The only thing I wanted to say, you should blame the artist for using self-harm as an aesthetic, not the person commenting. I agree too.

As someone who's had major problems related to the subject this is also making me feel severely uncomfortable, I personally don't think self-harm should be used together with "cute" either. My main character has scars but they are never shown as anything beautiful, good etc. People often it as a way to cope, try to accept themselves and their scars, but it is different.

It's frustrating to see it shown as something cute as it's trivializing the actual pain.

(If the artist replies that they self-harm themselves to excuse this - it does not matter in this case, it is the same as you don't make a cute character for aesthetics and don't give it, for example cancer because it fits the aesthetic.)
