I recently bought a clear case for my iPhone 4 so that I could cut out paper and use them as interchangeable backgrounds. This template fits the back of my phone perfectly! It will also fit the 4s.
There isn't a camera hole in the template, but here's what I do:
1. Place the paper into the case, design facing outward (like you would normally have it).
2. Using a pencil, trace the outline of the camera hole in the case.
3. Remove paper from case and use an exacto knife, coupon clipper, box cutter, or anything else to cut out the lines you traced.

Alternately, you can simply cut out a rectangle in the top corner so that the camera shows. I prefer to do this with black backgrounds, since the back of my phone is black and it blends in.

Don't worry about cutting the camera hole out perfectly; my first one was all weird and you can't even tell once it's in the case!
