Long story: A character named "Ditzy Doo" is mentioned but not shown on-screen.
A time later, due to an animation error, a background random generated character appears with crossed-eyes. Fans nick name her "Derpy Hooves".
Some time later, Word of God decides that "Derpy" would take over the role "Ditzy" would be.
Technically they are not the same character, as the 2nd was just fan-nicknamed over a random character, but many fans insists that they should be the same.
