Nice revamp of the NASA poster that was taped to my wall when I was a kid! I will point out two corrections.

The SIVB/Apollo stack went into orbit around Earth for, usually, 3 orbits while the spacecraft was checked out, and navigation was finalized. Then the SIVBs engine restarted at the correct moment in Earth to inject the stack into the Trans-Lunar trajectory. After that, the spacecraft lunar adapter panels separated, and the Command/Service Modules turned around to dock with the LM, and extract it from the upper end of the SIVB.

On the way back, after the Ascent Module of the Lunar Module rendezvoused and docked with the Command/Service Modules, and the crew transferred all the film and samples, and themselves, to the C/SM, the Ascent Module was jettisoned in Lunar orbit. The crew then fired up the big Service Propulsion Engine to accelerate out of Lunar orbit and onto a trajectory for Earth.

Very nice! This is the first time I've seen such a concise little chart of the whole mission.
