I felt like drawing some angst and it made me wonder... what would it take to break someone with such unwavering optimism as Milo? It wasn't until after I had finished drawing those last few panels that I realized...
What if Milo's brother died?
As a matter of fact, we only ever see Milo's younger brother in his concept art... what if he died tragically before the timeline of the game?! ...I mean, that's probably not true, but one can imagine.

I imagine Milo's probably the type to hide any negative emotions behind a smile, because he doesn't want to indirectly hurt anyone, even emotionally... hence the title.
