I'm not too knowledgeable on OCD, but I would like to say a few things (If they offend anyone or you, I'm sorry). The creature spewing out "WHO AM I?" greatly reflects those with OCD. It adds on that those affected with OCD may seem tidy on the outside, but are a wreck on the inside due to not thinking straight and struggling with identity. Also, the scratch texture n the background emphasizes the messy thoughts on just trying to make the littlest details perfect.

I appreciate very much your comment and I would like to reply (sorry for my English, I'm Italian).
I believe that people who experience OCD aren't wreck on the inside, I believe that they are people like everyone else that are suffering from internal conflict. Also, despite intrusive thoughts, it's possibile for them to think straight. OCD isn't necessarily about being tidy or struggling with identity, there are various themes in OCD (e.g. love, death, sexuality, religion...) but the root is only one, common to every sub-type: the unwillingness to feel or think what causes the internal conflict.
