Something like that. Its a meme in response to someone trolling you or doing something to make you angry, hence the term 'U mad?'

I'm not even mad is a response to that. the whole name is a reference to the popular image that is used with a caption of 'I'm not even mad' hence the file extension at the end.

TL;DR: It was a joke response to the girl having red(ish) eyes, which I'm guessing were meant to be a shade of brown.
