As for her eyepatch, I always assumed it had something to do with Anethesia's operation on her, like Rowdy Reiko's scar. Though I always thought Spencer's "heel" transformation more dramatic than the others, with no indication she would ever change back. Among Anesthesia's works, she was a masterpiece!

I've always felt Muriel already has deep identity issues. Anesthesia probably doesn't have to do much to give Mistress free reign. I also imagine she's one of Anesthesia's more dangerous experiments, more an ally than a minion, and sure to betray her at the soonest opportunity.
