Say, for a medical history:
Due to the limited space inside their body cavities, they were born with abnormally small chest organs, and their colons were connected to each other rather the exterior. To make way for healthier breathing, childhood surgery eventually eliminated one of their two livers, digestive tracks (so their two stomachs now connect to the same intestine) and 2 of their 4 kidneys, but as by then even the foals had mastered the tricky spell to vanish the contents of their colon and the alternative would have been lifelong adult diapers, they were left with no external opening.
Considering that neither of them has a rear end, they are both sexless. Regardless, each of them gained a sexual identity from early childhood, which has been carefully reaffirmed by many minor cosmetic surgeries to their faces.
Neurologically, they are also fascinating: as their spinal cords merge into each other, they share full sensation and control of their body except for each others' faces. As young foals they used to try and control their own half of their body and walk sideways, but as they grew up more they 'learned to share' and let the other one control their full body and walk forward. The merging of their spiral cords also gives them a weak mental link, which they describe as 'a weak background noise which is easier to tune out than to listen to'; through meditation lessons and lifelong practice they have learned to talk through it, but nothing else without actively focussing in it, and later getting horrible headaches.


And for a backstory:
Due to their inborn cardiac and respiratory insufficiency, they largely grew up inside hospitals. They quickly permeated hospital culture and decided they wanted to be doctors, assisting doctors and nurses to treat less fortunate fellow paediatrics patients. They have been cured from their insufficiencies, but habits die hard and they are used to avoid strenuous activities, thus leaning toward sedentarism.
Despite appearances, they do have cutie marks. They keep them under obscuring spells for two reasons: first, the marks display on each others' shoulders, so they keep them hidden so to not treat each other as a flank; secondly, one of their marks is much more strongly related to nursing than to direct medical practice, so they keep it hidden to not invite assumptions about one of them being the other's permanently attached nurse.

Likes: each other, medical practice, children, relaxing walks, levitating a good book while the other walks or cooks, spa visits, libraries, intellectual conversation, cooking for each other, sharing a cup of wine before going to bed.
Dislikes: the spell that flushes their colon, having disagreements, people who asks them if they want to be separated, people with vices, their cutie marks, physical exertion, people who flap their mouths rather than have intelligent conversations.

I believe this strange conjoined pony thing with a head on both ends is called a "Pushmi-Pullyu" (push me pull you). Or does that name only apply to llamas with heads at both ends? Oh well.
