Can someone please explain this to me? Who or what is Zeo? I understand that Zeo must be the dude in the bottom panel and he's doing whatever it is that made Brook's foot hurt but I feel I'm missing something.

if you haven't read about zeo by now, I will just say it. Zeo is one of the villains in the fishman island arc. a running gag with him is that whenever he is hurt/inconvenianced, he will claim he planned that (in a completely serious manner). When Brook stepped on him (long story) Zeo claimed that Brook didn't step on his face, instead Zeo was headbutting his foot.
so the joke here is that he really did headbutt brook's foot.

You'd have to watch One Piece after the 2 year arc to know who Zeo is. He's a fishman on Fishman island. He headbutted Brook's foot and it didn't affect him at the time. The comic is showing that it will affect him in the fuuuutuuuurrreee...

Brook accidently stepped on Zeo, and--in his ridiculous pride--he tried to pass it of as a headbutt to the foot on purpose. "You may not feel it now......but the throbbing will certainly come to discomfort you after some amount of time!!"
