The one she's glaring to is Screwloose, the crazy barking pony on the hospital from "Read It and Weep". The nurse is from that hospital.

Screwloose made a cameo in Games Ponies Play too. She was in the train, we can see her gruffy mane and azure ear very close to the camera when the mane six enter the train.

Read It and Weep. When the nurses chased the thief (Rainbow Dash), we heard a dog. Then when lights were on, we could see that there were no dogs, but a crazy looking pony (Screwloose) was barking and running with the nurses. "And you, go back to the hospital" one nurse says to her)

Probably because that would explain why he hasn't shown up much in the series, maybe some of the other pets have the same kind of double life. I don't know who their nemesis is though (perhaps King Sombra?)
