It's easy to call something out of the ordinary a mental disease. People used that to describe a lot about things they don't know a lot about.
How long was it ago when being homosexual was called a disease? As long as something is a fantasy I wouldn't say its illness.
Illness is when you know something is wrong and act on it.
Yet there is plenty of porn out there imitating acts that in general would be seen as absurd or immoral.
Especially in hentai and doujinshi.
Are you mentally ill for enjoying action movies where people get killed? That's what people used to say about comics, videogames and movies.
Also this scenario really implies how real animals would behave. Denying that snakes don't coil and eat prey is denying reality.

You can't explain fetishes. If you don't have one it seems nonsensical, but if you do they are very appealing. I know that's a say-nothing answer but it's just that simple. To a foot fetishist feet are sexy, to anyone else the very idea is ridiculous.
Vore can be explained as an extreme extension of the dominant/submissive fetish, though. It's hard to be more thoroughly dominated than when someone eats you.

This isn't a likely fetish to cause someone to die. Most vore fans want to be swallowed alive and there just isn't an animal around that could do it. Only one well documented incident of an adult man being swallowed by a snake exists and that man was dead when he was eaten. The animals that might be able to swallow a man alive like a giant grouper, great white shark, sperm whale or orca - are hard to come by.
So while creepy, vore is generally a harmless fetish. At worst the vore fan gets a snake and feeds things to it.
