Oops! I didn't want to spoil you! I've been reading the rest of the manga after the anime... he does have a human form as seen in this chapter, here on the bottom of the page [link] he pops up a few times in that chapter meep! I'm sorry!!!

eyes.. i think he have an eye colour if you see it you are scared to death XD something like.. yellow or so.

and hair.. uhm.. i think just black.
