Rose and all other gems can shapeshift. In her case, she made a womb to carry Steven. With that said, it's safe to say she made all the other reproductive organs necessary for procreation to happen. Gems don't naturally have any human genitalia, but they do identify with the female gender since they look, act like, and refer to themselves and each other as females.
