Yes you can use it as a real cursor. You need to:
1. Download it and save it as a .cur
2. Go to control panel (you probably have it on the start menu)
3. Go to mouse
4. Go to pointers
5. Go to downloads when you click browse (in home?)
6. Select which places you want to use your cursor for
7. Click apply and ok
