I think it means she must take care by herself because society is asking that, because if she doesn't care she would not look pretty, but in her heart she is not feeling well doing that, she's not happy because is an obligation. That's the meaning for me. Am I right?

there are so many things that work about this photo: the framing, the lighting, the expression; it achieves one of my favorite things about photography, it makes me feel, it is a perfect moment, although maybe a manufactured moment, but very beautiful and conveys much feeling.
