The hat on the coffin is the hat the scout always uses. The scout's mother (the woman weeping) is known for having an affair with the spy of the opposite team (seen here without a mask). The spy also has a bloody knife.
The title/ artist comments make up a quote taunt that the spy uses in the game "Here lies scout, he ran fast and died a virgin."
