You are right, I thought about adding spikes, chains and icons like they were ice cream sprinkles. But then I thought my friend (the comissioner) could not like if I changed the original model a lot.

It's simple. The heretic, tired of the punishement, gave a big mechanic middle finger to the Inquisitor, fleed to the nearest Cultist camp and joined them. They repainted the thing to be PIMPIN and doubled the drugs and stims doses.Done

In the fluff they are designed to profit from the crazed zealotry of the penitent while inflicting horrible pain to him, so I suppose they are expected to die early anyway
