I'll be honest. If I was in Luna's position, I would've finished the job and made it thorough. Pinkie Pie may have a condition, but that condition caused the death of others. It's easier to just end the problem right then and there rather than risk it coming back.

That’s the end? I can see this going one of two ways. 1. With her new disfigurement, Pinkie Pie will be able to start over and live a secret life under a new name. No one will be able to recognize her without her mane and tail, and her burnt skin will have her looking like a completely different pony. With her friends dead it’s not like she’ll be easy to identify. From there we can hear Pinkie’s side of the story, built on regret. Possibility 2. Surprise has an epic come back, with a creeepy new look from her injuries, and lust for revenge on Luna. There’s also a possibility that Surprise will use this new look to disguise herself in order to get closer to Luna. So many interesting stories can break off from this. I hope it doesn’t end.

Or it could be Luna who is seeking mental help after realizing how much she let her anger make her lash out at a pony who's only real crime was not being mentally sound to begin with.
