Poser7 + My Own Lights + much layers in photoshop.
I'll make a more specific tutorial when I have time, but I got the principles by which I light from here: [link] I first saw the link on the aery soul forum. It's not program specific, but you can kind of replicate it in poser.

I also made a little bit of a tutorial on faeriewylde here [link] about making a render pale - it's not exactly what I did here, but in general I tend to apply screen and overlay layers somewhat liberally.
